Not well.

元気ではありません。

と言ってみましょう。

how are you? の代わりに how’s it going? （元気？/調子はどう？）と言うと、フレンドリーでカジュアルになります。フレンドリーでカジュアルな答え方としては

Great!
や
Fantastic!
や
Really well!
とても元気だよ！

または

Fine.
や
Well.
元気だよ。

または

Okay.
や
So-so.
まあまあだよ。

または

Not well.
元気じゃないよ。

といったものがあります。
